Welcome to Niagara Endodontics! Your initial visit
includes a consultation to explain your diagnosis
and treatment options. Sometimes,
treatment can be done on the same day.
For complex cases, a follow-up appointment
may be needed.

Before Your Appointment

To ensure a smooth consultation, please prepare the following:

Required Information

Referral Slip and X-rays

If applicable, please bring any referral slips and recent X-rays from your previous dentist.

Medication List

Provide us with a list of all medications you are currently taking, along with their dosages.

Insurance Forms

If you have medical or dental insurance, please call in or bring in your insurance information.

Under 18 Years Old

For patients under 18, a parent or guardian must accompany them to the consultation.

Medical Conditions and Medications

Please inform us if you have any medical conditions or take medications regularly. This includes conditions like diabetes and high blood pressure, as well as heart medications or anticoagulants. Additionally, let us know if you require pre-medication before dental treatments.

About X-Rays

Transferring X-rays

If your previous dentist took X-rays within the last six months, you can request that they be forwarded to us. If this isn’t possible, please pick them up and bring them to your appointment.

Digital X-rays

If necessary, we can take additional X-rays at our state-of-the-art facility to ensure thorough evaluation and treatment planning.
